This award will recognise a team which has consistently given the very best service to their patients, clients or customers (who may be other departments).  The team will show true team working, with valued contribution from all colleagues at any level and any discipline, resulting in the best environment in which care or a service is delivered and for staff to work.

The entry must show:

  • they have used the principles of team working to achieve good practice;
  • they have provided an improved performance or service as a result of multi–disciplinary team working;
  • can demonstrate significant achievements and commitment; and
  • compassionate care

Nomination Guidance

  1. The citation should focus on the impact of the nominee.
  2. Please provide enough context for the judging panel to understand why the impact was significant as they may not have much knowledge of the field. Why does this work? 
  3. How are they going above and beyond what they are expected to do? What are they doing differently?

General advice: Start with a sentence that summarises the impact that the nominee has had, follow up with the achievement; provide evidence to support the case.
