Carers Passport Scheme

Carers are extremely important to our patients and know our patients better than most. While they are in hospital, the carer may still want to continue to support them and where possible we should enable them to do so. 

Working in partnership with carers can ensure that we provide the highest quality and most effective care possible. 

A carer does not have to be registered or provide care for a specific health condition. If someone identify themselves as a carer for a patient in our care, it is our duty to understand what support they provide to the patient and agree how we can enable this, if requested, in our hospital. 

The reason the patient is in hospital may be completely unrelated to the reason they have a carer but this does not mean the support of the carer is any less valuable during this time, particular those providing emotional support.

Our Carer Passport Scheme, aims to identify carers early and support them to continue aspects of the caring role, should they wish to do so. However some carers may wish to take some respite from their role whilst there are Health Professionals providing care.

Since the launch of the updated Carer Passport in June 2022, we have taken time to listen to Carers and their experiences. As a result please find below a myths busting factsheet based on what carers have shared with us.

Carer Awareness Training

We have also, developed a Carer Awareness and Carer Passport Training module, which you will be able to access on ESR form January 2023.
