Consent

The Clinical Effectiveness Team ensure as a Trust we are adhering to the purpose of the Consent for Examination or Treatment Policy, this is to ensure we have a coordinated and uniform approach to the implementation and monitoring of consent to examination and treatment and within the policy are the Trusts consent forms.

• Master copy of Paediatric form – Form 2
• Master copy of adults to lack capacity form – Form 4 - Please note: Following an MCA audit there has been slight amendments made to Consent Form 4. The changes are on the first page in sections B & C with bullet points being replaced with tick boxes which requires completion by the health professional proposing the procedure. 

The Clinical Effectiveness Team provides support to the Directorate Teams to develop and maintain any customised consent forms departments distribute to patients within their areas receiving treatment.

If your area would benefit from customised consent forms please contact the Clinical Governance Facilitator for your area and they will work with you to develop these. In addition to this they will ensure the customised forms are logged on the Trust database and liaise with supplies to ensure that the correct formats are being printed.
