What we do

The Specialist Palliative Care Team is an integrated team of specialist health and social workers.  The team works together to provide specialist advice and care to meet the needs of palliative and end of life care patients in both the  community and hospital setting within South Tyneside. 

The team manages complex health, social, psychological, spiritual and cultural needs that patients who have a life limiting diagnosis may experience, as well as being available to provide advice, help and support to medical, nursing and allied health professional staff.

The hospital specialist nursing and medical team is available from 09:00 – 17:00, Monday to Friday (excluding public holidays). The community specialist nursing, medical, occupational therapy, psychology and social work team are available from 09:00 – 17:00 Monday to Friday (excluding public holidays)  NB some elements of these services include part time staff.

The community Palliative Care Team and Out-of-Hours Palliative nursing team are available 24 hours a day including public holidays, facilitating urgent care, crisis management and avoidance of unnecessary hospital admission at the end of life.

End of Life Care Facilitators work across the hospital and community settings to co-ordinate the development, implementation and delivery of the end of life care agenda, as well as leading and planning audit and education.
