What we do 

Our Specialist Palliative Care Team is made up of healthcare professionals with a range of skills to help manage life-limiting illness. We work together to meet physical, psychological, social, spiritual and cultural needs of the patient, family and/ or carers. We are available to provide specialist advice and support to medical and nursing staff.

The End of Life facilitator is responsible to lead and co-ordinate the development and delivery of End of Life Care through evidence based practice; to develop, deliver and influence end of life care education and practice; plan and lead audits to monitor progress of initiatives relating to end of life care and provide regular reports. To project manage and lead the implementation of initiatives relating to end of life care.

  • For all HOOF Part A prescriptions please use our online form on our website www.dolbyvivisol.com - Click the Part A Hoof tab. Clinicans do not need to register and can submit a Part A Hoof to hoof.dv.nhs.net
