The Medical Records Department provides essential services to all Ophthalmic departments at Sunderland Royal Hospital, including our outreach clinics based at Durham Diagnostic & Treatment Centre, South Tyneside District Hospital, Hartlepool, Stanley, Peterlee, and Washington.

We provide the following services to patients:

  • Provision of case notes for outpatient appointments
  • Provision of case notes for theatre cases (inpatients and day cases)
  • Access to health records requests
  • Arrangement of all outpatient appointments
  • Front line reception cover, assisting patients booking into outpatient and emergency departments

Medical Records at Sunderland Eye Infirmary is built around five key areas of staff:

Outpatient Receptionists

Gemma Pownall, Higher Clerical Officer, supervises four outpatient reception areas (receptions A, B, Orthoptics, Durham Treatment Centre) and is responsible for booking all patients into their appointments and day case treatments, arranging further appointments for patients.

Emergency Department

Teresa Cook, Higher Clerical Officer, supervises the ED receptionists who provide cover daily between 8am and 8pm each day; they support the clinical team in ED by means of booking in patients and providing clinical notes for patients who are under follow up at SEI. Appointments and referrals arranged via the receptionists within a 24 hour turnaround.
