Overview

Our Care of the Elderly service is part of the Directorate of Rehabilitation and Elderly Medicine and provides an extensive range of services such as: acute elderly medicine, frailty, chronic heart failure, Parkinson’s Disease, delirium and dementia care (including Outreach Practitioners) and ortho-geriatric service.  The department also provides a Community Geriatric service which provides a responsive service to patients to avoid unnecessary attendance at the Emergency Department.

Sunderland Royal Hospital

Ward E51

Provides holistic, multidisciplinary care for acutely ill older patients. Dr Johnston provides specialist care for patients with Parkinson’s disease and movement disorders. The nursing staff are experienced in caring for patients with movement disorders. Dr Bateman, Dr Barnes and Dr Hosany also work as Community Geriatricians with an interest in community management so have close links with community services and with primary care.

Ward E52 

Provides holistic, multidisciplinary care for acutely ill older patients. The ward consultants have a specialist interest in caring for Frail older patients and form part of the Frailty Team. Dr Telford also provides specialist care for patients with Parkinsons disease and movement disorders. The nursing staff are highly experienced in caring for patients with movement disorders.

Ward E56

Provides holistic, multidisciplinary care for acutely ill older patients. The ward specialises in the care of the older person with cognitive impairment. The ward consultants have a specialist interest in dementia and delirium.

Ward B21

Provides holistic, multidisciplinary care for acutely ill older patients presenting with a range of conditions. The ward consultants have a specialist interest in Orthogeriatrics and Bone health.

Community Geriatrics

The Sunderland Community Geriatric service is a consultant led service that allows proactive management of frail older residents in Sunderland to help them stay at home and prevent acute admissions by:

  • Performing comprehensive geriatric assessments
  • Supporting use of advanced care planning
  • Improving communication between primary and secondary care
  • Provision of education and training to upskill the workforce supporting patients in the community

Based at Sunderland Royal hospital. We offer clinics around the city to allow patients to be seen nearer to home. Clinics are offered at the following locations:

  • Washington Primary care centre
  • Sunderland Royal Hospital
  • Seaham Primary care centre

We can also see those patients who are unable to be brought safely to clinic in their own home.
We work with existing community teams to optimise the care of Elderly patients in the community.

You are able to access the community geriatric team with a referral from your GP or from a City Hospitals Consultant Specialist Nurse or Therapist.
