Referral to Young Persons Contraceptive Nurse (YPCN)

The below referral form can be used for those who may have an identified vulnerability and who may not access mainstream services, who are < the age of 25 years with a focus on reducing Teenage Pregnancy in line with the current Teenage Pregnancy Action Plan, which is in line with the national teenage pregnancy prevention framework.
