CHILDREN REFERRAL- referral is made to the locality where the child resides.

South Tyneside Locality
Contact by telephone the Lets Talk Team on 0191 424 6000 in hours or 0191 456 2093 out of hours. A concern has not been raised until this team have been contacted and you have discussed the concern.

Sunderland Locality
If urgent contact Sunderland Local Authority on 0191 520 5552.

To send a referral to South Tyneside or Sunderland Local Authority via Datix Web

  • To create a Referral, select “Add a new Safeguarding Referral” from the drop down box
  • Please select "Safeguarding Adult"
  • Pick the local authority you are sending the form to and complete all sections in full
  • Click “SAVE” and then select section 9 on left of page
  • Click the drop down and select the appropriate Local Authority referral form. This will merge the details into the referral form and will be shown in the documents section. Double click the link to open the document for you to review and print for health records.
  • Select section 10, and select recipient from drop down (usually yours and manager name)
  • Type ‘local’ into the all users drop down box and select the correct email address of LA
  • Right click email text and copy and paste into message and amend with your details.
  • In the attachment section, you should attach your referral by clicking in the drop down box and selecting your referral.
  • Once you are happy with the referral, click “Send”

Gateshead locality

Contact, and submit your written referral form. Complete a Datix Web or Liaison. Save a copy of your referral and send to your safeguarding team
