Please find some useful information relating to personalised care conversations and approaches that have been taking place across the region.

These include primary care, secondary care and social care.

A virtual conference was held on Tuesday 21st September 2021 where many Personalised care approaches were showcased and it became clear that there are lots of pockets of work going ahead without other services knowing.

There is an opportunity for shared learning and collaborative learning.

This page can be used to share any personalised care approaches within your areas.

A further event is planned for early 2022.
