All applications for a basic mobile telephone and data contracts (for use in an iPad or laptop) should be made using the Application for Trust Mobile / Data Contract and in line with the Criteria for Mobile Devices.
Requests for basic mobile telephones for both sites will now be managed by Switchboard (0191 565 6256) at Sunderland Royal Hospital. Completed application forms should be sent via email to the Responsible Manager (Directorate or Business Manager) for appropriate authorisation. The authorised form should then be sent via email to stsft.switchboard@nhs.net
Requests for data contracts should continue to be submitted to the relevant IT Department (stsft.itservicedesk@nhs.net). Applications should be made using the revised Application for Mobile Devices Form, sent via email for authorisation by Divisional Director and then forwarded to IT for processing. If approved, IT will get in touch with the relevant individual to discuss requirements further.

When a member of staff leaves the Trust their mobile device should be returned to either Switchboard (basic telephones) or to the relevant IT Department. If user details have changed or a replacement handset is required, please complete the Voice only / Data Contract User Amendment / Replacement Handset Form. Please refer to the Process to request amendment to user details for voice only / data contract or to request a replacement voice handset for further details.
