The Acute Intermediate Care Team provides a nurse led service to the residents of South Tyneside. They provide urgent care to patients in their own homes as well as patients in Nursing and Residential homes.
Referrals are accepted from all health care professionals such as GPs, the ambulance service, nurses, social services as well as any member of the public.
We treat conditions such as
As well as treating these conditions we also deliver IV therapy for patients at home as well as supporting the District Nursing unplanned calls.
When the health professional or patient first contacts the service the call will be triaged and the call handler will ask questions to assess the condition of the patient. If a visit is appropriate the call handler will give an approximate time for the visit.
The team normally visit in twos and they will undertake a full assessment of the patient’s condition. Treatment may include antibiotics, nebuliser or pain relief.
The treatment plan will be discussed fully with the patient and the team will arrange follow up visits if required. The Acute Intermediate Care Team work alongside other Community Services that they will refer to if deemed appropriate, this would be discussed with the patient prior to any referral taking place.
