The Acute Intermediate Care Therapy Team is based at Clarendon in South Tyneside. The team consists of Occupational Therapists, Physiotherapists and Therapy Support Workers.
The team provides a 365 day service to respond to community crisis in order to prevent hospital admission as well as providing community rehabilitation to housebound patients and care home residents. The team also provide the therapy service to Haven Court for residential rehabilitation.
They work as part of an Integrated Care Team with Adult Social Care and Community Integrated Nursing Teams in order to provide care to patients in their own homes.
The team provide joint nursing and therapy assessments as part of the Intermediate Care Prototype, working in partnership with the CCG, Adult Social Care and a Community care provider to try and avoid unnecessary hospital admissions.
