The Unit has six private rooms, situated on the ground floor of Haven Court, which have been decorated to a high-specification and have an ensuite bathroom with a walk-in shower and access to its own calming garden area. Accommodation is also available for loved ones who wish to stay overnight.
The Cedar Unit is a nurse lead unit which has support from a local GP practice. Community palliative care team from South Tyneside and Sunderland Foundation Trust and specialist palliative care nurses are also available with support and guidance
Referral criteria:
All referrals must be informed that their admission is to an End of Life and Palliative Care Unit. The referral form (please click here and also available on EMIS) should be completed and sent to the below email address where it will be reviewed by the team. Referrals will be reviewed Monday – Friday.
The referral will be triaged with support of the Specialist Palliative Care team and the nurse in charge, who will contact the referrer with a decision.
If the referral is deemed urgent, please follow up with a phone call to the administration team on 0191 404 5500 who will escalate it to the nurse in charge.
All admissions to the unit will be between the times of Monday- Friday 8am- 3pm. For patient transfer, NEAS Palliative care ambulance should be used in majority of cases.
