We are a Community Integrated Care Team offering a range of services across Sunderland. We provide holistic nursing care to patients in their own home, preventing unnecessary hospital admission and allowing patients to be cared for in their own environment.

We provide nursing care for housebound people with long term conditions, acute and chronic conditions including end of life care.

We also provide wound care clinics in six areas across the city for patients with wounds who are not housebound

Our staff aim to promote independence and wellbeing by working together to provide people with the right care from the right professional at the right time.

Our teams consists of District Nurses, Staff Nurses, Health Care Assistants, Community Matrons, Older Persons Specialist Nurses, Social Workers and voluntary sector representatives.
