We are a Community Integrated Care Team offering a range of services across South Tyneside and Sunderland Community. Our aim is to provide holistic care to patients in their own home, preventing unnecessary hospital admission and allowing patients to be cared for in their own environment.
We provide care for housebound people with long term conditions, acute and chronic conditions including end of life care.
Our staff aim to promote independence and wellbeing by working together to provide people with the right care from the right professional at the right time.
Our teams consist of District Nurses, Staff Nurses, Health Care Assistants, Community Matrons, Occupational Therapy, Physiotherapy, Social Workers and voluntary sector representatives.
We operate a Single Point of Access for all referrals that are made into the service to ensure the patient receives the right care from the right nurse at the right time.

The community matron service in South Tyneside is now run using a Virtual Ward model.
This is goal orientated, aiming to increase patient knowledge and educate patients on how to best manage their long term condition with a view to reducing admissions to hospital and attendance at Accident and Emergency.
