We are a small specialist team based a STDH, providing assessments for those with permanent or long-term mobility problems. We prescribe buggies, manual wheelchairs and bespoke specialist seating according to clinical need. In collaboration with Regional Rehabilitation Engineering Service, we also provide powered wheelchairs.

We accept referrals from qualified health/social care professionals eg. Nurses, G.P’s, Therapists, and Social Workers. We have a short waiting list for powered wheelchair assessments; otherwise there is no waiting list.

Assessments take place in the most appropriate locations e.g. home, school, clinic etc. 
We will respond within three working days, to urgent referrals e.g. for patients with life-shortening conditions. Wheelchair QR JPEG.jpg

Please use this form to requiest a wheelchair or special wheelchair sitting item for clients who have a permanent disability.

To avoid delay in assessment/provision please complete all sections of this form, incomplete forms will not be accepted and will be sent back to the referrer.
