There have been a number of recent critical incidents whereby patients in our care have choked on their food.

On review, these patients had not received the correct consistency food, as recommended by SALT.

It is imperative that we:

  • refer patients to SALT if there are any concerns regarding their ability to swallow and/or eat and drink safely
  • communicate patients’ special requirements widely across the MDT and at handovers/huddles, so that all staff are aware
  • ensure patients’ families/carers are also aware of any special requirements
  • check that patients’ meals are of an appropriate texture, in line with the International Dysphagia Diet Standardisation Initiative (IDDSI)* as per SALT recommendations
  • provide the necessary support and assistance to patients who need help to eat and drink safely
  • escalate any new concerns to a senior member of staff / medical team.

*The IDDSI is an initiative which provides standardised terminology and definitions to describe texture-modified foods and thickened liquids used for individuals with dysphagia.
