Trust Patient Safety Priorities Survey

We are currently developing our Trust's Patient Safety Incident Response Plan (PSIRP) which will set out how STSFT intends to respond to and learn from patient safety incidents.

Within the plan, we need to identify our local patient safety priorities for investigation.

For this, we will use reported incident data, as well as data from inquests, claims, complaints and safeguarding enquiries.

However, we would also like to gather "softer" intelligence from our staff.
