The Child Health Information System (CHIS) supports the safe and effective delivery of the childhood public health programmes, including Childhood Screening, Immunisation, Public Health and Healthy Child programmes.  The Child Health Record is the only full Public Health record of a child and in our area this is held on EMIS. The CHIS Team will ensure that the right information is recorded in the right format, recorded only once and available to the appropriate clinical and non-clinical staff.  The CHIS Team provide monitoring, failsafe and population surveillance for the whole child health and community electronic patient record. We are contracted directly by NHS England to hold a full record for every child under our care and it is vital to be given all relevant information for that child from all partner organisations including 0-19yrs service, GP’s, Hospitals, Screening Labs, Maternity services etc.

  • Child Birth or Death
  • Have you discovered a child or family who have moved in or out across Sunderland, South Tyneside or Gateshead areas?
  • Immunisation, Screening and Healthy Child Programme information
  • Seen or know of a child record that is not accurate, for example, duplicate record, changes to name, address, NHS Number, school or GP
