A SAR is a review involving more than one organisation. It looks at how things could have been done differently to prevent harm or death. It aims to promote learning and improve practice.
A SAR can take place in the following circumstances:
South Tyneside Safeguarding Children and Adult Partnership (STSCAP) has published two briefings.
The names of the people involved have been removed. This is to protect their identity. The full versions are available on the STSCAP website by clicking here.
The new reports refer to an adult referred to as AP and adults detailed as AS and AT.
The briefings include a number of areas of learning and questions to think about in the future.
