The Security team would like to remind all staff that all violence and aggression incidents must be reported on Datix in a timely manner and with sufficient details of what occurred. The Datix report should give clear detail of what exactly happened and state whether the patient(s) involved were warned about their behaviour more than once.  If you provide this information and it’s clear that the patient(s) continued to disregard the warnings given then it should support the decision making process in awarding a risk marker against the patient. Please also ensure that all of the perpetrator details and victim details are completed to assist with investigation. Once you have submitted your Datix report, please ensure you keep check on any responses you receive about the incident as this may be requesting details about whether you want a risk marker submitting for this patient.

Also when submitting a security report for violence and aggression or verbal abuse it is very important that we understand whether there was any clinical conditions or aggravating factors that may have contributed to the incident.  We read too many reports that state no for both but when reading the incident detail it states that the patient was elderly and confused.  This contradicts the information you have previously recorded by saying no to any factors been present that may have caused the incident.
