Red 2 Green - discharging patients

When confirming a patient is medically fit for discharge (via the Red 2 Green assessment)  you must tell us what assessment/services are needed for a safe discharge. We also need to know who has asked for it/recommended it. 

You must document any conversations you have with the patient/relatives, family, carer in relation to discharge planning and pathway agreement. We cannot progress referrals without consent.

If you call the IDT please state the patient's name and not the bed/bay number. You will also need to know which pathway the patient is on. This is so we can direct the call to the correct person
