We are re-launching End of Life Comfort Care Packs at Sunderland Royal Hospital.

Comfort Care Packs are provided to our families when a patient approaches end of life and remains in hospital for end of life care. The patient should have commenced the Care of the Dying Document by the Medical Team and Nursing Teams are to complete the EOL Holistic Assessment , which triggers and evidences the question around 'has a comfort pack been provided?'

Each pack comprises of;

  • Cushion   
  • Fleece Throw
  • Plastic sealed bag containing paper cups, stirrers, coffee sachets, tea bags, sugar, whiteners, cup a soup, biscuits, hand & face wipes, toothbrush & toothpaste, pen & paper.

All above are packed into a re-usable bag for the family to keep.

The process during normal working hours - Ring the Medical Library on extension 47400 or End of Life Care Facilitator Karen Crammond on 07786028923 (Mon- Fri 8am-5.30pm and Saturday & Sunday 8am-1pm), to request a comfort care pack.

Outside of those hours, including weekends & bank holidays, staff are to contact a Porter for access to these.

Each comfort pack needs to be signed for by person collecting to include the below info; 

  • Date
  • Patient's Name
  • Ward
  • Quantity (maximum 2 packs per person)
  • Signature
