Substance navigators can support patients who attend with substance use offering harm min advice, referrals to community alcohol and drug services and any interventions needed identified on an individual patient need . They do NOT offer prescribing advice- this still needs to come from community alcohol/drug services ie: wear recovery/STARS.

Please refer patients Via V6 under ACT and include reason in the free text box if drugs.

Hours of work are Monday Friday 9-5 but referals can still be made outside these hours - a telephone follow up call will be made.

Navigators can do assertive outreach and still make contact with patients if they are discharged before being seen in hospital.

ACT STDH will be starting March 2023  (referrals can be submitted from April 2023) 

ACT SRH will be starting  May 2023  (please do not refer Sunderland patients until June 2023)

V6 referrals pathway:
Go to Orders, search for ACT and complete Eletronic referral. If patient needs to be seen urgently or for discharge, please BLEEP ACT  (numbers above) as we are not in office 24/7. If OOH please refer on V6 and we will contact patient in the next 24 hours.
