Following your feedback, we are making some minor but really important changes to the observation chart in Meditech. This will be live from Thursday 28th July
 
The first to ensure that escalation of deteriorating is in line with our escalation policy.
  • The observation chart will identify if a single vital sign triggers a score of 3 this will automatically populate​
  • A trigger as above will mandate that the escalation section must be completed​
  • Additional text will indicate the correct use of the escalation section. (to note person to escalate to will depend upon role of person documenting, not all options as below)
 
 
 
The second change is to add a Capillary blood glucose monitoring plan, having this monitoring plan will give you visible on laptop status board of your patients' plan to ensure these vital observations are not missed. This will not be a mandatory item as not all patients will need this investigation.
