Following the re-launch of the Carer Passport last week during Carers Week, the Patient Experience Team will be visiting wards and departments to deliver resources to help staff support the scheme.

The Carer Passport can only be issued by a Ward Manager or nurse in charge to an identified carer.

Each inpatient ward will receive a Carer Passport resource box.

The kits include card holders and lanyards and pens, as well as a the agreement.

A leaflet has also be produced by the team and can be read by clicking here.

The introduction of a Carer Partnership Agreement booklet will provide written documentation for the carer of the reasonable adjustments that have been agreed.

The Carer Partnership Agreement and Carer Passport is only valid for the ward the patient is on and therefore must be revisited if the patient is transferred or discharged then readmitted.

However, a Carer Pass can be issued by supporting teams who identify a carer involved in a patient's care.

This pass recognises the carer and will be kept by the carer to show to staff during appointments or emergency admissions.

This will enable them to assist the patient as best possible during that episode of care.

Information regarding Carer Awareness and Carer Passport Training Sessions will be available next week for staff to book onto and we would encourage all staff, clinical and non clinical to attend.
