Following the introduction of the QR codes to collect Friends and Family Test Feedback, it would appear that our response rates across the Trust remain low. 

Whilst it is great to see the Ward staff attaching the QR postcards to the discharge information, this does not appear to be reflective in the numbers. Therefore if we could encourage all inpatient wards to utilise the CIVICA app on the iPads so the survey is completed prior to discharge this may be a solution. The CIVICA app should take you direct to the survey for your ward, however if this is not the case and you are asked to filter options to find your department or are having any other issues please contact the team who will be happy to help.

Work is still ongoing to get the CIVICA app on all community devices, however, there have been a number of issues encountered. If your device, is not showing the app or you would like to try and complete the installation contact the team who would be happy to support.

The Friends and Family test still remains to be a national requirement and a Trust standard. If your team is having technical issues and the patient does not have their own device please continue to offer the paper survey as required to ensure we are still obtaining patient feedback until we are fully online.
