District Nursing referral via Meditech

A message from Melanie Robertson, Assistant Director of Nursing:

I am delighted to inform you of an improved process. Ffrom Wednesday 15^th February, referrals to South Tyneside and Sunderland District Nurses should be made via Meditech.

Please see SOP and user guide for details of this new process. 

When the appropriate D/N referral document is completed in Meditech and saved, this will automatically send a referral via nhs.net to the Community Integrated team.

Some specific details to be aware of:

• For urgent D/N referrals where a patient needs a visit in <2 hours, the referrer should telephone SPOC in addition to completing the Meditech document
• D/N referrals outside of theses localities (eg. Durham and Gateshead are not made via Meditech at present and existing processes should be followed)
• The Meditech document auto-populates from existing nursing and AHP assessments. Please ensure that these are up to date before making referral. Inactivating assessments such as IV devices will prevent completed assessments being visible on the referral.
