The Wound Assessment Template on Meditech is changing. From Tuesday 14th June, there will be 3 separate assessments to choose from: 

  1. If a patient has Moisture Associated Skin Damage (MASD), the MASD Assessment template must be completed. 
  2. If a patient has a pressure ulcer, the Pressure Ulcer Assessment template must be completed.
  3. If a patient has a different wound, the Wound Assessment template must be completed.
